How Hospice Can Help

Considered to be the model for quality, health care at the end of life, hospice care involves a team-oriented approach to expert medical care, pain and symptom management, and emotional and spiritual support, tailored to an individual’s health care needs and wishes. Support and guidance is also provided to a patient’s family members. Members of the hospice staff make regular visits to assess the patient and provide additional care. Hospice staff are on-call 24 hours a day, seven days a week.

What services are provided? The hospice team will help:

Manage the patient’s pain and symptoms

Address the patient’s emotional and spiritual needs

Provide medication, medical supplies and equipment

Coach the family on how to care for the patient

Grief and bereavement support for surviving family and friends

When should I ask about Hospice?

Decline in medical condition
